Brain Aneurysm

Brain Aneurysm Treatment: Coiling, Clipping or Flow Diverter

There are three main ways to treat a brain aneurysm. This short explains the differences between surgical clipping, endovascular coiling, and using a flow diverter. The choice of treatment depends on the aneurysm's size, location, and shape.

What is a brain aneurysm?

A brain aneurysm is a weak or thin spot on an artery in the brain that bulges out and fills with blood. The goal of treatment is to prevent it from rupturing.

  • Rupture risk. An aneurysm can leak or rupture, causing bleeding into the brain. This is a stroke and a medical emergency. Treatment aims to stop blood from entering the aneurysm sac.
  • Unruptured aneurysms. Many aneurysms are found by chance during scans for other conditions. These are called unruptured or incidental aneurysms.
  • Watchful waiting. Not all aneurysms need immediate treatment. Small, unruptured aneurysms that are not causing symptoms may be monitored with regular imaging.

What are the main treatment options?

The two main approaches are open surgery (clipping) and less invasive endovascular procedures performed from within the blood vessel.

  • Surgical clipping. This is an open surgery where a surgeon places a tiny metal clip at the base of the aneurysm to seal it off from the artery. It requires opening the skull (a craniotomy).
  • Endovascular coiling. In this minimally invasive procedure, a catheter is guided from an artery in your groin or wrist to the brain. Soft platinum coils are then packed into the aneurysm to block blood flow.
  • Flow diversion. This is another endovascular technique. A mesh stent is placed in the parent artery across the aneurysm's opening. This diverts blood flow away from the aneurysm, allowing it to shrink over time.

How is the right treatment chosen?

The decision is based on the aneurysm itself, your overall health, and a discussion of the risks and benefits of each approach.

  • Aneurysm factors. The size, location, and shape of the aneurysm are key. For example, wide-necked aneurysms can be harder to treat with coiling alone.
  • Patient factors. Your age, general health, and whether the aneurysm has ruptured are all considered. The risks of open surgery may be higher for some patients.
  • Team decision. The choice is made by a team that includes a neurosurgeon and an interventional neuroradiologist. They will explain the best options for your specific case.
  • Recovery. Recovery is usually faster after endovascular procedures. However, clipping may offer a more durable, long-term solution for certain types of aneurysm.

The most suitable treatment for a brain aneurysm depends on many individual factors. A final recommendation can only be made after a specialist has reviewed your case, including your medical reports and recent brain imaging.
