Liver Cancer

Liver Cancer Surgery in India - Liver Resection vs Liver Transplant

Surgery is a primary treatment for liver cancer. This short explains the two main surgical approaches: liver resection and liver transplant, and how a specialist decides between them.

What is a liver resection?

This surgery, also called a partial hepatectomy, removes the part of the liver containing the tumour.

  • When it is suitable. Resection is an option when the cancer is contained in one area and the remaining liver is healthy enough to function and regrow.
  • The procedure. A surgeon removes the tumour along with a margin of healthy tissue. The liver has a unique ability to regenerate to its original size over time.
  • Key advantage. This approach preserves your own liver and avoids the need for lifelong immunosuppressant medication that is required after a transplant.

What is a liver transplant?

A transplant is a procedure to replace your entire diseased liver with a healthy donor liver.

  • When it is considered. A transplant may be an option for certain early-stage cancers, especially if the liver is also badly damaged by a condition like cirrhosis.
  • Eligibility criteria. There are strict guidelines regarding the size and number of tumours to be eligible for a transplant for cancer. This helps to ensure the cancer has not spread.
  • Donor livers. The new liver can come from a deceased donor or, in some cases, from a living donor who gives a part of their liver.
  • After surgery. You will need to take medication for the rest of your life to prevent your body from rejecting the new organ.

How is the decision made?

  • Tumour factors. The size, number, and location of the tumours are critical. The decision also depends on whether the cancer has grown into nearby blood vessels.
  • Liver function. Tests are done to assess the health of the liver tissue that would remain after a resection. If you have advanced cirrhosis, a resection may not be safe.
  • Your overall health. Your general fitness for a major operation is also an important factor in deciding the best course of treatment.

The choice between resection and transplant is complex and depends on your individual case. A specialist can determine the most suitable treatment for you after a full evaluation, including a review of your medical reports and recent imaging.
