What is Total Hip Arthroplasty?

Total hip arthroplasty, or total hip replacement, refers to a surgical procedure that involves the removal of a diseased hip, and replacing or substituting it with an artificial hip joint. An artificial or prosthetichip is comprised of a ball and socket, and is compatible with the human body. Artificial hips are made to withstand wear and tear, and to resist degradation and corrosion. Total hip arthroplasty is one of the most successful surgeries performed with excellent 15–20-year survivorships routinely reported in large series by multiple surgeons. Over the past few years, modified surgical techniques such as capsular repair, the use of alternative bearing surfaces, and minimally invasive techniques have become increasingly popular. It is widely used in the treatment of severe hip osteoarthritis, avascular necrosis, rheumatoid arthritis, gout and other general arthritic conditions.

Who needs Total Hip Arthroplasty?

Total hip arthroplasty generally is performed in patients who have damage to their hip joint, either from injury or hip arthritis. When used in conjunction with physical therapy, Total hip arthroplasty often can relieve pain. The procedure also can help restore function and range of motion to the hip joint.

Hip replacement surgery usually is performed after other non-surgical treatments have failed. If pain medications, exercise, and physical therapy fail to relieve hip pain and restore range of motion, total hip replacement may be a consideration. Typically, medical conditions that may necessitate total hip replacement are bone tumors, hip fracture, and osteonecrosis, which is a condition where blood supply to the ball of the hip is inadequate. Many times, osteoarthritis and rheumatoid arthritis also might require a hip replacement.

Total Hip Arthroplasty Surgery Procedure

Total hip arthroplasty is a surgical procedure for replacing the hip joint. This joint is composed of two parts - the hip socket (acetabulum, a cup-shaped bone in the pelvis) and the "ball" or head of the thighbone (femur). During the surgical procedure, these two parts of the hip joint are removed and replaced with smooth artificial surfaces. The artificial socket is made of high-density plastic, while the artificial ball with its stem is made of a strong stainless metal. These artificial pieces are implanted into healthy portions of the pelvis and thighbones and affixed with bone cement (methyl methacrylate).

An alternative hip prosthesis called a "cementless" Total hip arthroplasty has the potential to allow bone to grow into it, and therefore may last longer than the cemented hip. This is an important consideration for the younger patient. In some cases, only one of the two components (socket or stem) may be fixed with cement and the other is cementless and would be called a "hybrid" hip prosthesis.

How long will I be in the hospital after surgery?

After Total hip arthroplasty, the hospital stay usually ranges from three to four days. Patients who live with someone who can assist them are usually able to go home without much problem. However, patients who live alone are encouraged to find someone who can stay with them when they return home. Patients who do not progress well may require a short stay in a rehabilitation center, where they can receive physical and occupational therapy and instruction in how to care for themselves at independently.

How successful is Total Hip Arthroplasty Surgery?

Results of Total hip arthroplasty are generally very good; most people are relieved of their entire hip and groin pain and the replaced joint will usually last ten years, depending on patient use and activity levels.

The major long-term problem is loosening of the prosthesis. This occurs either because the cement crumbles (as old mortar in brick building) or because the bone melts away (resorbs) from the cement.

Loose, painful artificial hips can usually, but not always, be replaced. The results of a second operation are not as good as the first, and the risks of complications are higher.
